End your season the right way at the US Champions Cup National Championship. This youth tournament will be held at the MoneyGram Soccer Park in Dallas, Texas and is open to boys and girls U9-U19. US Champions Cup National Championship is the most prestigious soccer championship in the country.

Application & Refund Process:

Upon acceptance into the tournament, your team entry fee must be paid in full immediately to guarantee your spot. Entry fees are non-refundable. In the event your team must withdraw from the tournament, you must notify the tournament director in writing at least 30 days prior to the tournament date and you will be allowed to transfer your entry fee to a future event.

All teams are required to stay in one of the official tournament hotels and use US Champions Cup’s approved travel provider to compete at the event. See details under the Travel Packages and Rules tabs.

Events may be subject to availability, capacity limits, eligibility requirements, and may be subject to delay, change, or cancellation for inclement weather. Application deadline subject to change due to volume of applications and event capacity. In the event an Age Division Bracket does not attain an adequate number of registered teams, the Tournament Director shall notify those registered teams as soon as possible. The teams will be given the option of playing up or receiving a full refund.

Advance Online Check-In

All teams attending the US Champions Cup National Championship are required to participate in advance electronic registration by submitting their required documents online to their GotSoccer account.

Important: On-site Team Check-in is required. A team representative will need to pick up the team's credentials and sign to verify that all information received for your team is accurate and current.

Traveling Teams

All teams are required to stay in one of the official tournament hotels and must make their reservations through Travel All Season International, Inc. Please note: Failure to make your reservations through US Champions Cup approved travel vendor can result in the non-acceptance of your team.

All teams, except as noted below, are subject to book lodging through the US Champions Cup approved designated travel provider as a condition to compete at the event.

Teams located within a 100-mile driving distance of MoneyGram Soccer Park are not required to book lodging.

Addresses used for the determination of the distance/time will be the address of MoneyGram Soccer Park and the team's primary contact address.

Google Maps TM will be used to determine driving distance and time.

Checklist

Please follow the below checklist to complete your registration and prepare to compete:

Book room accommodations with the approved US Champions Cup travel vendor, Travel All Seasons International, Inc.

All Teams: Travel through Travel All Season Travel by calling 972-392-9800 or visiting www.travelallseasons.com.

Due Date: June 7, 2019
If you do not complete your room reservations by this date your team will not be scheduled and will be cancelled from the tournament.

Waivers

All of your participants appearing on the roster (coaches, athletes, staff, etc.) must sign an Event Waiver & Medical Release Form which are located in the Required Documents Section.

All of your participants appearing on the roster (coaches, athletes, staff, etc.) must sign an event WAIVER OF LIABILITY, RELEASE, ASSUMPTION OF RISK AND INDEMNITY. Download the Waiver and Medial Release and have parents (if player/participant is under 18) or player/participant complete the waiver and upload to GotSoccer.

Online Roster

MANDATORY: All teams must enter their rosters complete with bench personnel into the tournaments on-line scheduling system no later than 21 days prior to start of event. The minimum required information is Name, Date of Birth, player ID number, uniform number and gender. Revisions can be made to the roster up the night of team registration. This information will appear on the tournament game reports to be used by the referees for check-in on the field prior to each game. Teams failing to input this information are subject to removal from the event.

Sanctioning/ Player Cards
USYS AND US CLUB SOCCER SANCTIONING

This tournament is sanctioned by USYS and US CLUB SOCCER. In order to participate, all players and coaches are required to have a current US Youth Soccer or US Club Soccer or their Affiliates Player/Coach Pass Card. If you are not a member of any of these organizations please call (800) 4-SOCCER to obtain membership information from USYS.

SANCTIONING RULES US YOUTH SOCCER OR US CLUB SOCCER

In order to participate in this tournament all USYS AND USCS teams must be in "Good Standing".

SANCTIONING RULES FOR NON-TEXAS TEAMS

In order to participate in this tournament all USYS and other USSF affiliates must be in "Good Standing" and obtain Permission to Travel* from their State/National Association. Please bring a copy of your completed Approved/Certified Roster along with your Permission to Travel to the on-site registration.

Application to Host form is in the Required Documents section.

*Permission to Travel is not required from teams registered with US Club Soccer.

In order to participate in this tournament all teams from outside the United States must have written permission and verification from US Soccer and their country's Federation indicating authorization to participate in the US Champions Cup Tournament. Please bring a copy of your completed Approved/Certified Roster, your Permission to Travel and Proof of Insurance to the on-site registration. International teams (players and coaches) will be required to present passports at registration.

In order to participate in this tournament all teams from outside the United States must have written permission and verification from US Soccer and their country's Federation indicating authorization to participate in the US Champions Cup Soccer Tournament. We need to change US Champions Cup Soccer Tournament to US Champions Cup National Championship.

6. Waivers

7. Medical Release

Medical Release Download US Champions Cup Soccer events shall require all teams participating in the tournament to provide medical releases for each player in an appropriate form. Most organizations/clubs/leagues/teams have their own forms. These may be acceptable for US Champions Cup Soccer events. These authorizations shall be reviewed by 2012 Sports Events | US Champions Cup at registration and kept in the possession of a team official with a copy to US Champions Cup representative.

MoneyGram Soccer Park is a 120-acre, 19 field soccer complex at Elm Fork in Dallas and is owned by the City of Dallas. The facility features 14 full-sized fields & five junior fields to provide areas for all age groups. The venue hosts various events on both the local and national levels. Such events include tournaments, league play, clinics and the occasional team training session. Conveniently located to the west of I-35E on Walnut Hill Lane, the complex is a tournament-grade facility that will enable the sport of soccer to continue its growth in North Texas.
